It’s easy to request a medical record from a patient visit at any of our hospitals, Freeman Urgent Care, Freeman Heart & Vascular Institute, or any other Freeman facility. Just follow one of the following steps:

Additional Forms & Records

With the recent update to the HIPAA Privacy Rule (45 CFR parts 160 and 164), certain types of requests for protected health information (PHI) that may involve reproductive health information (RHI) now require an attestation.

These specific categories of requests are:

  • Health oversight activities
  • Judicial and administrative proceedings
  • Law enforcement purposes
  • Disclosures about decedents to coroners and medical examiners

If your request falls within one of these categories, please ensure that the appropriate attestation form is completed and sent to Medical Records using one of the submission methods indicated above. You can access the attestation form here.
